Dear New Patient,
Please read the following information carefully and let us know if you have any questions:
- Please download and fill out the New Patient Questionnaire and bring it to your first appointment. If you do not have time to do this at home, please arrive 15-20 minutes before your scheduled appointment to fill it out in our office.
- Your first visit will include a thorough health history and examination as well as your first acupuncture treatment. Please allow 1.5 hours. Your subsequent appointments will take approximately 1 hour.
- Please dress comfortably or bring loose-fitting clothes to wear during your treatment.
- Do not brush your tongue or wear any make-up or perfume for your appointment. Your natural appearance is an important part of the diagnosis.
- Please bring copies of any recent medical reports, lab results or any other pertinent information about your medical care.
- Please bring complete information about your current and recent medications, including names, dosages, prescribing doctors. The same applies to any over-the-counter medications or supplements you may be taking.
- It is suggested that you have a light meal before the treatment. Avoid coming to your appointment without having had any food for several hours.
- Payment is due at the time of treatment. We accept cash, checks as well as MasterCard and Visa.
- Please keep in mind that we require 24 hours’ notice to cancel any scheduled appointment. You can leave a message by phone or e-mail to avoid being charged in full for your appointment.
